Integrity, safety, and accountability are of the utmost importance. Please review all information attached concerning CLSN Therapy policies and procedures, processes, and services. Fill out all of the attached patient history and background information before the scheduled assessment in order to expedite the process.

All information will be used to create an appropriate treatment plan that will allow for success now and in the future.

Evidence-Based Approach

What is ABA?

Applied Behavior Analysis (ABA) is an evidence-based approach to creating meaningful or socially significant behavior change. New skills and behaviors are taught while existing behaviors are modified.

ABA focuses on behaviors that are observable and measurable, with respect to their function. This is determined through the collection of data that involves antecedents and consequences, which are events that occur directly before and after the behavior of interest.

This approach utilizes principles of reinforcement, to increase skills that are functional and socially significant throughout the child's daily life. ABA not only teaches these skills, but also promotes maintenance and generalization.

What is Floor-time/Play Therapy?

Floor-time, also known as Developmental, Individual-differences, Relationship-based (DIR) Floor-time, is an intervention that focuses on promoting emotional and intellectual growth in children with developmental and learning challenges.

It is a play-based therapy that aims to help children develop the foundational capacities for relating, communicating, and thinking. Floor-time encourages parents and caregivers to engage with the child in a way that follows the child's lead and interests.

Types of Services CLSN Provides

Center-based Therapy

Our center-based program focuses on teaching basic skills to enhance learning for each child. Each child participates in a session with a Program therapist or therapist technician, with individual programming created by the Director.

Naturopathic services integrated

Home-based Therapy

To ensure all skills are generalized, our home-based services include training parents or caregivers to take on the role of the therapist under our supervision. This provides holistic support in natural environments.

Family-centered approach

Functional Behavior Analysis (FBA)

Designed for children with behaviors interfering with learning. Analysis completed via parent interview and direct observation, with naturopathic services integrated for holistic approach.

Data-driven interventions

Parent Training

All therapy services include parent training component. We recognize that parent involvement is crucial for lasting effects. Naturopathic education available for comprehensive support.

ABLLS-R

Assessment of Basic Language and Learning Skills Revised

Criterion-referenced assessment for language, academic, self-help, and motor skills. Provides task analysis of skills and individualized curriculum development.

FBA

Functional Behavioral Assessment

Primary tool to identify and understand behavior. Identifies specific target behaviors and their purpose to develop intervention strategies.

VB-MAPP

Verbal Behavior Milestones Assessment

Criterion-referenced tool for children with autism. Five components: Milestones, Barriers, Transition Assessment, Task Analysis, and Placement/IEP Goals.

FEDL

Functional Emotional Developmental Levels

Assesses child's functional emotional development across six DIR levels: Regulation, Engagement, Shared Attention, Two-Way Communication, Symbolic Thinking, and Logical Thinking.

All therapy services are FREE and at NO COST to families.

Rules and Regulations

Scheduling And Sessions

  • Sessions typically scheduled in 2-3 hour blocks for optimal skill retention and mastery
  • Parents should arrive 10 minutes prior to session end for consultation with therapist
  • 30 days notice required for significant scheduling changes, vacations, or termination
  • Sessions include direct services, material prep, data collection, and parent discussion time

Absences, Vacations And Holidays

  • CLSN provides calendar of scheduled vacation days and holidays in advance
  • Leaves of absence or extended vacation require 30 days notice and Director approval
  • Arrangements made on case-by-case basis upon approval

Illness Policy

  • Temperature 100°F+: Child must be picked up immediately and fever-free for 24 hours without medication before returning
  • Diarrhea: Two+ instances requires pickup; return only after 24 hours symptom-free
  • Vomiting: One+ instance requires pickup; return only after 24 hours symptom-free
  • Common Cold: Clear runny nose and slight cough permitted; colored discharge prohibits attendance
  • Rash: Doctor's note required stating non-contagious status
  • Communicable Diseases: No attendance for pink eye, impetigo, hepatitis A, scabies, ringworm, chicken pox, strep throat, etc. Doctor's clearance required to return

Observation Of Client

  • Video recording may occur for staff training and progress updates; all videos kept confidential
  • Professionals, trainees, and other clients may observe therapy with prior notification
  • Parents may view therapy sessions and may be asked to observe for generalization promotion

Medical Information

  • Medical and psychological records release required for treatment planning
  • Permission granted for CLSN to seek emergency medical assistance when necessary
  • Certain medical conditions and medications (e.g., insulin) may require specialized staff; services may be limited if staff cannot safely administer

Cancellation Policy and Fees

If written notice for cancellation of a session is not received 24 hours prior to the scheduled session, documentation for a no call/no show will be applied.

If more than 3 no call/no shows are documented, the fourth and any cancellation thereafter will result in a ₦10,000.00 fee. This ensures consistent and quality service are provided to our clients.

Service Agreement

This document contains important information about our professional services and business policies. It also contains summary information about the Health Information and Portability and Accountability Act (HIPAA), a federal law that provides privacy protections and client rights.

Although these documents are long and sometimes complex, it is very important that you read them carefully and that you ask questions you have about the procedures at any time. When you sign this document, it will also represent an agreement between us.

Individual behavioral and skill assessments, short and long-term therapy service provision to youth in the autism spectrum and related developmental areas.

Appointments

Scheduled appointments are important. Please give as much notice as possible for cancellations due to illness or emergency to allow us to offer your time to another person.

Confidentiality

All services are confidential except with written authorization or as mandated by Federal Republic of Nigerian law and professional codes of conduct/ethics.

Client Protection

Mandatory reporting for suspected abuse of minors, elderly, or disabled persons. Protective actions taken if client threatens serious harm to self or others.
